Patient Information For our current patients we look forward to updating your record
when you check in for your appointment. If this is your first visit
to our office, we invite you to fill out the forms listed and bring
them with you. We will be making a copy of your insurance card, so
please bring this as well. If you have a co-payment please be aware
that it will be collected at the time of your visit. For your convenience,
we accept payment in the form of check, cash or MC/VISA.
In order to provide a thorough evaluation, we recommend you bring
a list of all medications you are currently taking, and a list of
previous surgeries. To ensure you get the most out of you appointment,
we encourage you to bring a diary of any symptoms you are experiencing
as well as a list of questions you may have. We look forward
